       Forms can be printed off and mailed to
PHSA during sign ups, click MYSA Registration form on page for a 
printable form Fees are:

U-5 through U-8 are $45.00
  U-9 through U-12 are $55.00
    U-13 through U-16 are $60.00

Roster sizes are mandated by MYSA.
   Teams will be filled on a first come first 
       served basis, after the deadline or a team
       fills up we will try to place all players on a team at the boards discretion.
